Letter to Your Doctor

Once you’ve completed your Advance Directive and Durable Power of Attorney for Health Care, you’ll need to create a letter to your doctor or doctors to request that the document become a part of your permanent medical records. Sign this letter and mail it or deliver it to your doctor along with a copy of your advance directive and durable power of attorney for health care.

To download and print the letter to your doctor:

1. Click on 'Your Documents' at the top of the program.

2. Scroll down and select 'Letter to Your Doctor - Review/Print'. 

3. You can review your letter here, then scroll down and select "Download & Print"
