Here's a short summary of the steps you should take once you have completed the program:
1) Review and Print
Review and print each of your documents. You can find printing instructions here.
Changes can be made on the review page by clicking the item you'd like to update.
- Your Revocable Trust
- Your Will
- Your Advanced Directive Durable Power of Attorney for Health Care
- Your Financial Power of Attorney
- OPTIONAL Letter to your Doctors
2) Notary and Witnesses (everyone needs their state ID or Passport)
Each state has different requirements therefore please read the instruction sheets included to direct what needs notarized and/or witnesses on each document.
3) Final step
Will
Distribute copies to named beneficiar(ies), executor and keep original in safe location (Protection Portfolio)
Advance Directive Durable Power of Attorney for Health Care (ADDPOAH):
Create letter to your doctor and letter and copy to your doctor(s) to become part of your permanent medical records, agent(s) and keep original in safe location (Protection Portfolio).
Financial Power of Attorney(FPOA):
Keep original in safe location (Protection Portfolio). Do not distribute and tell agent where they can find it.
Revocable Trust:
Funding your trust (changing ownership of assets to the trust) there are instructions and forms under Your Documents > Revocable Trust> Funding Your Trust. Also, included in the funding area is a Certificate of Trust which will need to be notarized before sending to establishments. Once everything has been funded put original document in a safe location (Protection Portfolio) and notify your Trustee where to locate it.
